The following fees are only "estimates". Actual prices may vary depending
on the complexity of the treatment.  Several factors, including but not
restricted to: patient health, bone density and thickness, type of implant
required, bone grafts, and bone regeneration, may result in increased fees.
These factors will be assessed during your complimentary consultation and
prior to surgery.

If you are considering dental implants as an option to loose / worm dentures or missing teeth,
then you should ask these Four Questions of your Dental Practitioner.
1.    How long have you been placing and restoring dental Implants, and do you perform both
       the surgical and prosthetic phases?
2.    How many implants have you placed?
3.    What is your success rate? and
4.    What are your qualifications in Implant Dentistry?
At the Implant Smile Center, we feel it is our obligation to provide you with that information.
Dr. Leigh has limited his practice exclusively to Dental Implant Surgery and Prosthetics.

ESTIMATED FEES FOR 'STANDARD' DENTAL IMPLANT PROCEDURES:

 DENTAL IMPLANTS: $1,000.

IMPLANT SMILE CENTER NEW 2008 FEE SUMMARY:                            TOTAL FEES

1. Single Dental Implant (Root Form) with Implant Crown (Complete)                             $2,500.

2. Two (2) Root Form Implants with Ball Abutments Attached (
NO DENTURE)             $2,800.
3. Two (2) Root Form Implants with Locator Abutments Attached (
NO DENTURE)     $3,000.

4.  Four (4) Root Form Implants (Mandible) with a Gold Implant Bar

     with Bilateral Cantilevered Sections Attached (
NO DENTURE)                     7,000 to $8,600.

5.  Five (5) Root Form Implants (Mandible) with a Gold Implant Bar

     with Bilateral Cantilevered Sections Attached (
NO DENTURE)                                  $11,000.

6. * Six (6) Root Form Implants (Mandible) with a Screw-Retained (Hybrid)
      Fixed/Removable Appliance Attached.  (Completed Case)                                         $15,000.

The above FEE SCHEDULE is for Standard Implant Placement Cases ONLY (assuming there is
sufficient bone width and height) where 3.5 mm. diameter and wider implants can be utilized and
DOES NOT include : Radiographs, Diagnostic Wax-ups, Study Models, Surgical Guides, Temporary
Prostheses, Extractions, Oral Hygiene Instruction or Bone Grafting Procedures when/if required.
(The majority of these procedures are not required for standard implant cases)

It DOES include a Free Consultation, 2nd Opinion, Fee Quotation and minor Alveoplasty/Osteoplasty
procedures where required. Complex cases may incur additional charges.

We also offer COMPLIMENTARY ONE EVENING OVERNIGHT ACCOMMODATION AT A WESTLOCK
HOTEL
the evening prior to the Implant Surgery Procedure, for the convenience of our patients if
requested (minimum of 2 implants required).

We believe these to be  the VERY BEST PRICES IN ALBERTA at present (well worth the short trip to
WESTLOCK). We sincerely hope that this New Fee Schedule will help all patients appreciate the
tremendous benefits of Implant Dentistry and make Implant Dentistry more accessible to Albertans.

All Prices are in Canadian Dollars.

Bone Grafting (fees vary due to size and type of grafts).

When comparing fee estimates, patients must keep in mind that there are two phases to
dental implants:
1.    The Implant Phase. Placing the implant and healing collar in the mouth.
2.    The Prosthetics Phase. Placing the abutment in the implant, and attaching the crown.
Some dentists do only one of the phases -- either the implants or the prostheses. This may
lead to greater overall cost. Other dentists, like Dr. Leigh, do both phases of the procedure.

It is also important to choose an implant practitioner who has the training, expertise, and
experience to place various types of implants, and perform a multitude of implant
procedures, such as bone augmentation, ridge splitting, and bone grafts. Follow-up
aftercare is also a major factor in successful implant surgery.

Fees may change without notice until a final treatment plan has been arranged and
accepted; following which, the arranged fees will be valid for a period of 90 days.

Although the final results of Dental Implant Treatment, as well as all other dental treatments,
cannot be 'guaranteed', Dr. Leigh will stand behind the dental work he performs at the Implant
Smile Center. Failed 'standard' implants will be replaced at no additional charge, or your money
refunded. During the first five years of function, Crowns or removable appliances (overdentures)
will be repaired if broken or replaced as necessary for no additional fee. This does not include
the routine replacement of metal or nylon retention clips or caps, which must be changed as
wearing occurs.

We encourage you to be an informed consumer when considering Dental Implants. The
Implant Smile Center offers a "Complimentary Consultation".  Please contact our office for
